Residency Affidavit

Since the Residency Affidavit must be notarized, it is only available in PDF format.

  • Download and print the Residency Affidavit (58K PDF).
  • Complete the form and have it notarized; mail it to the address listed on the form.

The Residency Affidavit was formerly known as the HB1403 affidavit. (This form is NOT to be completed by U.S. citizens or U.S. permanent residents.)
